Request for Assistance (7.59KB).
This form is used by the Outreach Committee. It is used to record all the information pertaining to a request for assistance by an individual or family. Forms may be submitted to the Church Secretary or Pastor.

WUMC Day Camp Enrollment Form (14.42KB).
The Daycamp Enrollment form can be submitted to the Church Office, Pastor, or Preschool Director.

WUMC Purchase Order/Reimbursement Form (10.73KB).
A purchase/reimbursement form must be completed and submitted to the Church Treasurer in order to obtain a reimbursement from the church. Submit the fully completed form to the Church Secretary or Treasurer.

Employment Applications can be submitted to the Church Office or Pastor.

WUMC Preschool Enrollement Form (14.42KB).
The Preschool Enrollment form can be submitted to the Church Office or Preschool Director.

Church Van Trip Report (7.59KB).
Trip reports are to be filled out and placed in the Trip Reports Folder in the Trustees mailbox.

This documment outlines the Guidelines for using the Church Van.

Church Van Usage Report (21.15KB).
Church Van usage reports are to be completed for each trip and submitted to the church secretary. Reports may be left in the secretary\'s door tray.

Permission Form and Medical Release (13.47KB).
This form is to be completed by all Youth before taking part in any Youth Trip or Activities.

This form is to be completed by anyone wishing to use the Church Facility. The completed form should be submitted to the Church Pastor, a Trustee, or to the church office.
